-The start of NCHSAA fall sports is delayed until at least September 1.
-The first five (5) student days of the 2020-2021 school year will be designated as a “dead period” for ALL sports, allowing school staff to focus on the start of school.
-NCHSAA Phase One of the summer conditioning and workouts will continue until further notice.
The North Carolina High School Athletic Association (NCHSAA) and Davie County Schools are requiring student-athletes to complete the following:
-Physical Examination Form (See PPE form specific information below based on the examination date)
-Initial Screening Question Form
-Gfeller-Waller Concussion Awareness Form
-DCS Acknowledgement of Risk, Waiver of Liability Regarding Voluntary Participation in Athletics and Other Activities
NCHSAA INITIAL SCREENING TO PARTICIPATE IN ATHLETIC ACTIVITY DURING COVID-19 FORM (PDF)
All student-athletes students wishing to be involved in athletics are required to complete this form before being allowed to participate in ANY organized activity
NCHSAA PREPARTICIPATION SPORTS EXAMINATION 2020-2021
NCHSAA NOTE - ABOUT THE PREPARTICIPATION SPORTS EXAMINATION
All student-athletes who had a valid Preparticipation Examination (PPE) form dated anytime between March 1, 2019 – May 1, 2020 (used for the 2019-2020 seasons) will be granted a temporary one-year extension through the end of the 2020-2021 academic year. If the student does not have a valid form from these dates, they will need an updated physical form before any summer workout or season participation.
DATE OF CURRENT PHYSICAL | REQUIREMENT FOR 2020-2021
-Before 03/01/19 | Need valid, up-to-date physical before participating.
-On or after 03/01/19 through 05/01/20 | Granted a temporary extension to participate through the end of the 2020-2021 academic year, but the student-athlete and parents/guardians are required by Davie County Schools to complete the history portion of the PPE.
-On or after 05/02/20 | Will participate based on the current policy of being “valid for 395 days”, but the student-athlete and parents/guardians are required by Davie County Schools to complete the history portion of the PPE.
STUDENT-ATHLETES WITH A VALID PPE FORM DATED on or after MARCH 1, 2019 - MAY 1, 2020
All student-athletes who had a valid Preparticipation Examination (PPE) form dated anytime between March 1, 2019 – May 1, 2020 (used for the 2019-2020 seasons) will be granted a temporary one-year extension through the end of the 2020-2021 academic year. Davie County Schools requires student-athlete and parents/guardians to review, fill-out, and sign this portion of the form before participating in any summer workout or season participation.
STUDENT-ATHLETES WITH PPE FORM DATED before MARCH 1, 2019
All student-athletes who have a Preparticipation Examination (PPE) form dated before March 1, 2019, are required to have an up-to-date physical before any summer workout or season participation
